How attendees describe this talk
It is a helpful training on how to learn from an incident with a neutral approach on asking why and how do we do it better. And in that neutrality encouraging helpful feedback from employees.
Measure the distance between what work is Imagined to be Vs. How the work is actually done. Make that distance as small as possible.
Fantastic information and presented in a manner that was clear and concise with usable tools.

Feedback for Jake and the event
I wish more power points were like this, the inclusion of QR codes to link data was great. As a small business sometimes it is difficult to translate high level information into a manageable format that could fit our needs, I felt the clear steps visually detailed in the power point made it much easier. My only suggestion would be to add how a process like this could be applied to the small business.
There is an element of importance in taking responsibility for individuals. While not necessarily for discipline, it’s important for employees and people to own up to mistakes.
This was one of the best I've seen.
